This role will be covering following towns - Old Saybrook, Old Lyme, East Lyme, Waterford, New London, Groton, Stonington and surrounding towns
Job Description
- Facilitate communication between the patient and the interdisciplinary team.
- Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
- Utilize state-of-the-art technology to document skilled services provided.
Qualifications
- Licensed Practical Nurse with Valid CT LPN license
- One year of LPN experience; Homecare experience preferred.
- Valid driver's license and reliable transportation required.
- Computer proficiency required.
